$PEPE #HSE On April 19, 2025, at 17:00 hours, Mr. Rashid Khan, an Insulator with Employee ID 40045807, climbed up to a height of 6 feet via ladder. Upon reaching the scaffolding platform, he started his work but didn't notice the gaps between the scaffolding planks and one missing plank. When the safety personnel spotted the hazard, they immediately instructed Mr. Khan to descend. Fortunately, no injury or incident occurred.
